We offer individual therapy, FMLA and medical leave assessments, academic accommodation evaluations, emotional support animal (ESA) evaluations and letters, and gender-affirming care documentation. All documentation is based on an individualized clinical assessment.

Services are available to eligible clients in Georgia, North Carolina, Massachusetts, California, and Montana, subject to licensure, service type, and applicable requirements.

Documentation assessments and related paperwork are private-pay services. Insurance may be accepted for eligible individual therapy appointments through participating platforms.

Documentation services start at $125. Any additional fees will be explained during your appointment. Documentation time is generally not separately billable to insurance.

You will meet with a licensed clinician to discuss your needs, relevant history, and documentation request. Recommendations are based on your individual circumstances and clinical findings.
